Deadline for Athletes is August 20, 2015. Any athlete who has not turned in required paperwork and paid appropriate fees by 3:00 pm on August 20th will be removed from athletics for the remainder of the first semester.
Requirements for grades 9-12
1. Athletic physical and information packet on file in Athletic Trainer's office.
2. Sudden Cardiac Arrest Information sheet signed and on file in Athletic Trainer's office.
3. Drug test taken through DATL.
4. Copy of Drug Contract on file in Athletic Director's office.
5. Signed copy of Parent Guidelines on file in Athletic Director's office.
6. $25 athletic fee paid.
Requirements for grades 7-8
1. Athletic physical and information packet on file in Athletic Trainer's office.
2. Sudden Cardiac Arrest Information sheet signed and on file in Athletic Trainer's office.
3. Signed copy of Parent Guidelines on file in Athletic Director's office.
